Download and complete this form to register your child to use the short-break services provided by The Oak Foundation.

As well as providing your child's details, you'll need to provide the name of their GP plus details of any medications they take, including times and dosages.

Please also let us know if your child has any special dietary requirements and the cues to look out for if they have no verbal communication.
